Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued.

This is a request for quote (RFQ), and the solicitation number is 36C25223Q0510. The Government anticipated awarding a firm-fixed price Purchase Order resulting from this solicitation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-07 (eff. 9/10/2021).

The associated North American Industrial Classification System (NAICS) code for this procurement is 339112, with a small business size standard of 1,000 employees. The FSC/PSC is 6515.

The Department of Veterans Affairs, Network Contracting Office 12, is soliciting quotes from all sources to supply the Clement J. Zablocki VA Hospital, Operating Room, 5000 West National Avenue, Milwaukee, WI 53295-1000 for Arthrex Trimano Fortis Support Arms and Adapters or Equal to expand their Surgical Services to the Outpatient Surgery Center (OSC). This requirement is being requested as a Brand Name Only. Respondents to this solicitation must fully demonstrate their capability by supplying detailed quote information, along with any other documents necessary to support the requirements below.

Quotes are to be provided to Jennifer Mead, Contracting Officer, via email at Jennifer.Mead@va.gov, no later than April 10, 2023, by 09:00 Local Time. No telephone calls will be accepted. No submission received after the above-mentioned date and time will be accepted.Â
